Consent to Treat

Our office policy is that a parent or legal guardian must accompany a minor (less than 18 years of age) at all times. This is our policy for several reasons:

  • To assure that the information we obtain is correct.
  • To allow you to hear our findings.
  • To have you available to consent to tests or immunizations/procedures.
  • To allow you to learn about the expected course and what complications to watch for.

In today's world of working parents, it is sometimes necessary for nannies, friends or relatives to bring a child in to be seen. It is also sometimes necessary for an older teen of driving age to come by himself or herself. In these cases, please complete a Consent to Treat form or a Power of Attorney form.

Phone Calls

Our Triage Nurse is available Monday through Friday at (480) 860-8488 to answer baby care questions, medication questions, or to help you decide if your child needs to be seen. If you wish to speak to the doctors, non-emergency calls are answered at the end of the afternoon and, as time permits, between appointments. You may also message your primary care doctor when logging into the patient portal. For a complicated problem, please schedule an appointment.

Medication Refills

Our patient portal is designed for you to request your prescription(s) online when you message your primary care physician. You can also have your pharmacy call our office for medication refills. When calling, please specify dose, time given, and quantity desired. Please allow 1-2 days for processing.

Payments

Our policy is to collect payment in full for services at time of the visit, including all co-pays due. For your convenience, we are pleased to accept cash, check, MasterCard, VISA, Discover or American Express. If you need to arrange a payment plan, please contact our billing department.

Fees

Charges for professional visits, procedures, medications and lab tests are determined based on the time spent, the cost of supplies, and the complexity of the problem.

Scheduled consultations or well visits cancelled with less than a 24-hour notice will incur a $50 charge billed to you; missed appointment charges are not covered by insurance plans.

Insurance

For patients who have conventional insurance, we provide a copy of the “Attending Physician’s Statement” for you to submit to your insurance company for reimbursement.

For patients on one of the HMO/PPO plans to which we belong, our contract requires that we submit insurance forms upon receipt of your co-payment (if required) at the time of the visit.

Hospitals

For newborn care, we use Honor Health Shea Campus. If you plan to deliver at another hospital, ask about our arrangement with other pediatricians there.

For hospitalized care, we use Honor Health Shea Hospital or Phoenix Children’s Hospital.

Emergencies

We recommend that all parents learn CPR and keep the numbers below handy:

LIFE-THREATENING EMERGENCIES – call 911 for the paramedics.

Other true emergencies – go to your nearest emergency room (Scottsdale Healthcare Shea Hospital). Ask the ER staff to contact our office upon arrival.

Poisonings

First, call the Poison Control Center at (602) 253-3334. Then call our office.

After-Hours Care

For urgent problems that can’t wait until the next day (no medication refills or routine questions, please!), call the office at (480) 860-8488, and the answering service will contact the nurse or doctor-on-call. When you call, describe the problem and indicate whether you need your call returned immediately or whether it can wait an hour. Please unblock your telephone by pressing *87! Have your pharmacy number ready, and make sure they are open. You may turn the blocking function back on after talking with the physician or nurse by pressing *77.

Urgent Problems

Call our office at (480) 860-8488. If we are out of the office, the answering service will contact us. If an immediate response is needed, please say, "This is an emergency!"

Back-up Physician/Triage Nurse

For urgent concerns that cannot wait until regular office hours, always call our number, (480) 860-8488. A Triage Nurse affiliated with Phoenix Children’s Hospital may be on-call for routine problems. There is always a physician back-up if the problem warrants or if you are not comfortable with the response provided by the Triage Nurse.

Developmental Consultations

For children with suspected problems of behavior or learning (hyperactivity, speech delays, motor coordination problems, learning disability, etc.), we offer extended consultations. Please be aware that some insurance plans have separate benefits for behavioral health, and you may be responsible for additional costs.
