Who can accompany my child to the office?
A visit with the doctor is an important and sometimes scary event for a child. Our office policy is that a parent or legal guardian must accompany a minor (less than 18 years of age) at all times. This is our policy for several reasons:
- To assure that the information we obtain is correct.
- Allow you to hear our findings.
- To have you available to consent to tests or immunizations/procedures.
- Allow you to learn about the expected course and what complications to watch for.
In today's world of working parents, it is sometimes necessary for nannies, friends, or relatives to bring a child into be seen. It is also sometimes necessary for an older teen of driving age to come by him or her self.
To help accommodate these occasions we have provided two exceptions to our policy:
- A signed, faxed "Consent to Treat Today" form, sent by the parent with a valid phone number where the parent can be reached during the appointment. The parent MUST be available. Download Consent to Treat Today Form
- If the parent will not be available (out of the country, etc.) during the appointment a "Power of Attorney" form must be filled out and brought by the caretaker to any appointments while the parents are unavailable.
